SAGGING SKIN AND NON-ABLATIVE CUTANEOUS RADIOFREQUENCY In an era particularly polarized on beauty and skin care, non-ablative radio frequency (NARF) represents one of the most modern techniques for the treatment of aging and sagging skin of face and body, which can be rejuvenated in a non-ablative manner without the use of needles or drug injections. The NARF was the first non-surgical method to be authorized by the Food and Drug Administration (organization that ensures the safety and effectiveness of electro-medical tools and products launched in the U.S. market) for the treatment of facial and body skin laxity. The principle of this method is based on producing a controlled and homogeneous increase in temperature in the deep dermal and subcutaneous tissues by modifying the electrical charges, and subsequent cell movement of electrons, by applying radiofrequency energy to the tissues. The amount of heat produced varies depending on the amount of energy applied, the impedance of the tissue (the tissue resistance to the passage of RF current), and the duration of the application. This heating effect is used by NARF to induce skin rejuvenation. In particular, the temperatures reached in the deep dermis, induce a rearrangement of the collagen structure, which becomes shorter and thicker, and an increase in fibroblasts proteosynthesis with greater production of collagen and elastin. All this translates into an increase in consistency of the dermis, and a marked tensor effect appreciable on inspection and palpation.

Several areas of the face and body can be treated: around the eyes, nasal furrows, cheekbones, forehead, neck, decollete, arms, inner and outer thighs, buttocks and abdomen. The treatment lasts from a minimum of 15 min. to a maximum of 30 min., and allows immediate resumption of normal daily activities without sequelae, or specific post-treatment requirements.

The results (restoration of tissue firmness, thickness, firmness and radiance) are not immediate, but are gradual and progressive. The treatment protocol provides a series of 10 sessions with a frequency of once or twice a week. The results for facial rejuvenation are visible immediately after the first session and last for 8-12 months. In other locations the results are achieved more slowly (about 1 month after the first session) but still last more than a year.

The NARF has many advantages over other methods used in the recent past: it is a well-tolerated treatment (patients have often described a pleasant feeling of warmth and relaxation), it is not painful, and is suitable for males and females of all ages and skin types, at almost any time.


In un’era particolarmente polarizzata sulla bellezza e sulla cura della pelle, la Radiofrequenza non ablativa (RFNA) rappresenta una delle più moderne tecniche utilizzabili nel trattamento dell’invecchiamento e del rilassamento cutaneo di viso e corpo, ai quali dona un aspetto riposato e giovane in maniera non ablativa e senza la necessità di utilizzare aghi o farmaci da iniettare. La RFNA è stata la prima metodica non chirurgica ad essere autorizzata della Food and Drug Administration (organo che garantisce la sicurezza e l’efficacia di strumenti elettromedicali e farmaci immessi sul mercato americano) per il trattamento del rilassamento cutaneo di viso e corpo. Il principio di questa metodica si basa sull’applicazione ai tessuti di una energia a radiofrequenza che attraverso la modifica delle cariche elettriche cellulari e il conseguente movimento degli elettroni, produce un aumento della temperatura in maniera controllata ed omogenea attraverso il derma fino alla fascia muscolare esclusa. La quantità di calore prodotto varia in funzione della quantità di energia applicata, dell’impedenza del tessuto (la resistenza che ogni tessuto oppone al passaggio di corrente a radiofrequenza) e del tempo di applicazione. È proprio questo effetto termico che viene utilizzato dalla RFNA per indurre il ringiovanimento cutaneo. In particolare alla temperatura che si raggiunge nel derma profondo, si verifica un riarrangiamento della struttura del collagene che diviene più corto e più spesso, un incremento proteosintetico dei fibroblasti con conseguente maggiore produzione di collagene ed elastina. Tutto questo si traduce in un aumento di consistenza del derma e un marcato effetto tensore apprezzabile all’esame obiettivo sia ispettivamente che palpatoriamente.
La metodica si esegue facendo scorrere un manipolo sul piano cutaneo, e prevede il trattamento di diverse zone del viso e del corpo : contorno occhi, solchi naso-genieni, zigomi, fronte, collo, decolletè, braccia, interno ed esterno coscia, glutei e addome. Il trattamento dura da un minimo di 15 min. ad un massimo di 30 min., e consente una immediata ripresa delle normali attività quotidiane senza postumi o particolari prescrizioni nel post-trattamento.
I risultati non sono immediati ma si evidenziano in maniera graduale e progressiva, riuscendo a ridare ai tessuti tonicità, spessore, compattezza e luminosità. Il protocollo terapeutico prevede un ciclo di 10 sedute con una frequenza di una o due volte alla settimana. I risultati per il ringiovanimento del viso sono visibili già dopo la prima seduta ed hanno una durata di 8-12 mesi. In altre sedi i risultati si ottengono più lentamente (circa 1 mese dalla prima seduta) ma permangono per oltre un anno.
La RFNA presenta numerosi vantaggi rispetto ad altre metodiche utilizzate in un recente passato, è un trattamento ben tollerato (spesso i pazienti hanno descritto una sensazione di calore piacevole e rilassante) non è dolorosa, adatta a soggetti di entrambi i sessi, di tutte le età e fototipi, e praticabile in qualunque periodo dell’anno.
